• OSBI form (PDF): Background check form. Applicant MUST fill in all blanks that are highlighted. OSBI background checks are free. Submit forms to Janel Melton in Human Resources 587-0291, 587-0660 fax.
  
  
• Tutor Information Form (MS Word): This form will help you collect personal information from all your volunteers. From phone numbers, to e-mail address to place of employment, you'll be able to keep accurate records of your volunteers using this form.
  
  
• Tutor Preferences (MS Excel): Use this Excel spreadsheet to track your tutor time/day preferences, as well as, the assigned student and their classroom teacher.
